Cryotherapy with liquid nitrogen
  • cotton-tipped swab or spray for 10-15 sec until a 'frost ball' extends 1-2mm beyond the margins of the lesion. A blister will later form that dries & then sloughs in about 10 days.

Mild strength corticosteroids:

  • Hydrocortisone 1 - 2.5% cream, lotion, or ointment

  • Desonide (Tridesilon) 0.05% cream, lotion, ointment

Mid -strength corticosteroids:
(for eczema or resistant rashes)

  • Fluocinolone (Synalar)  0.01 - 0.025% cream, 0.035% ointment

TOPICAL ANTIFUNGAL RX:
  • Penlac (Ciclopirox) topical 8% solution for onychomycosis   NEW!  9-2004
    * Loprox (Ciclopirox)1% lotion BID for fungal nail. $15/1 oz
  • Lotrimin/Clotrimazole 1% cream OTC $4.80/30g, 1% soln $6.80/10 ml

PO ANTIFUNGAL RX:
  • Lamisil (Terbinafine) 250mg/d x3month
  • Sporanox (Itraconazole) 100mg cap 2 cap bid aftermeal x 1 wk,then rest for 3 wks (repeat x 2)  pulse dosing , or 2 cap daily for 12 weeks for onychomycosis.
  • Nizoral (ketoconazole) 200mg 1 tab/d for 2wks
  • Griseofulvin: Fulvicin P/G 125-165-250-330 tab     330 or 375mg /day.

   

TOPICAL ANTI-INFECTIVES RX:
  • Bactroban (Mupirocin) 2% ointment TID 15-30gm
  • Betadine (Povidone-iodine) soln 4-8-16-32-128 fl oz
  • Garamycin 0.1% cream/ointment 3-4x/d
  • Silvadene (Silver sulfadiazine) 1% cream 20-50-85-400-1000gm 1-2x/d
  • Neosporin cream (PolymyxinB, Neomycin)Neosporin oint (Above + bacitracin)
  • Polysporin oint (PolymyxinB+bacitracin)
  • pHisoHex (Hexachlorophene) cleanser 0.25- 5 oz, 1 pt
  • Vioform-HC cream (Clioquinol 3% + HC)

ACNE Rx:
  • Accutane (Isotretoinon) Retinoic Acid 10,20,40mg cap 0.5-2mg/kg/d in BID
  • Benzamycin topical Gel BID (3% erythromycin,5% benzoyl peroxide),
  • Cleocin (Clindamycin) 1% lotion,soln
  • Erythromycin 1.5-2% topical soln BID
  • Desquam-E 2.5, 5 or 10 Emmolient Gel (Bezoyl Peroxide) BID
  • Desquam-X 2.5, 5, 10 Gel or Wash
  • Retin-A Micro 0.1% gel (20,45 g tube)Retin-A 0.1% cream

 
Anti-Scabies & Lice/Pediculosis Rx:
  • Permethrin 5% cream (Elimite)
  • Lindane 1% lotion or cream (Kwell) apply from head to toes for 8 hrs.
  • Atarax 25 mg for itching
  • Topical steroids or Calamine or Caladryl lotion

 
For Actinic keratosis Rx:
  • Efudex /Fluorouracil 5% cram or 2 or 5% soln bid for 2-4 wks.
